Dear Settlers Life beneficiary:
Please accept our condolences on your loss of your loved one. Rest assured that Settlers Life is here to assist you in your time of need. We know that at the time of a loved one's death, having life insurance proceeds quickly and readily available to pay for final expense can help relieve the stress of an emotional event.

To start the claim process, you may call our Claims Department at 1-800-334-9607 for a claim form, or you can simply click here for one.

The claim form will need to be completed, and signed by the appropriate beneficiary(s)*. Please return all originals to us, along with a certified death certificate with cause of death and the applicable policy(s).

If the beneficiary(s) wishes to assign the policy proceeds to a funeral home, in addition to the above claim form, we will require a signed, notarized assignment form. Simply give the assignment form to the funeral home director for completion. Please note the beneficiary(s) must sign the assignment for policy proceeds to be paid to the funeral home. If additional information is needed, the Claims Department will contact the beneficiary(s) directly. Likewise, after the claim is processed, the beneficiary(s) will be notified by mail. Again, if we can be of further assistance to you; do not hesitate to let us know. *One claim form is required for each designated beneficiary.

Save time and money by having your policy premium automatically withdrawn from your bank account. Many times when you choose EFT your premium rate will be lower!

Here's all you need to do to have your premium payment automatically withdrawn from your bank account:

  • Click here to download the form. You will need the free Adobe Acrobat Reader to view and print it.
  • You can fill out the form electronically or by hand.
  • Print the form. Please make sure you've signed it.
  • Fax the form to 276-645-4398 or mail the form and your voided check to:

    Attn: Policyholder Services
    PO Box 8600, Bristol, VA 24203-8600
    In order to protect your personal banking information please do not email the completed form to us.
  • In order to have premium payments withdrawn from your bank account, the policy payments must be up to date.

Filling Out The Form Electronically

  • Check the boxes you want by moving your mouse over the box and clicking (the cursor will turn from a hand to an arrow). To un-check a box, simply click on it a second time.
  • To type on a line, click where you want to start and begin typing. You can use the tab key to advance to the next line.
  • Once you've filled out all applicable parts, print the form by clicking on the print button at the top of the screen (part of Acrobat Reader).

Note: When you close Acrobat Reader, your entries will not be saved. If you print the form and realize you need to add or change something, your entries will still be there as long as you did not close the program.
