Request For Service

The Request for Service Form will allow you to request a copy of a lost policy as well as change your name and address, mode of payment, and beneficiary.

To submit a Request for Service form, please follow these instructions:

1. Click here to open the form. You will need the free Adobe Acrobat Reader in order to view and print it.
2. Print the form and fill in the sections that are appropriate to your change.
3. Please make sure you've signed the form and then mail it to the address at the top of the form.
4. You will receive a letter confirming the changes you requested.

Note: In order to make these changes, we require a signature; this is for your protection and for this reason, we need to have you print, sign and mail this form to us.
